Mahmee, an integrated care delivery platform for maternal and infant health, is seeking a Care Coordinator to join our team and support our growing patient population. This job is all about advocating for new and expecting parents, identifying patients’ needs and creating opportunities for personalized support and education, and staying involved and engaged throughout the pregnancy, birth, and postpartum journey.
 
Work Schedule and Work Location:
This is an independent contractor position with a starting commitment of 10 hours per week, split into 2-3 hour daytime shifts scheduled at your availability. We envision this role growing over time into a part-time or full-time position. This position is remote.
 
Compensation:
This is an independent contractor position paid at an hourly rate of $17-23 per hour commensurate with experience, and an opportunity for the position to grow over time for the right candidate. This is a remote role.

Primary Responsibilities:

  • Be the “Switchboard Operator” for new and expecting families: promptly respond to questions in the Mahmee platform and through other customer support channels, and route questions as needed.
  • Assist patients with exploring the Mahmee Network, building their Care Teams, setting up their Medical Profiles and Care Plans, and getting fast access to care through New User Welcome Sessions, and 1-on-1 interactions with patients.
  • Offer referrals as needed to Mahmee Network providers, community-based programs, classes and groups, and other resources available in the Mahmee platform.
  • Perform Care Coordinator intake and follow-up calls that help patients to identify healthcare needs and find solutions in the Mahmee Network.
  • Assist with booking and scheduling of patients for Mahmee-led groups, classes, and VRN appointments.
  • Provide product-related support to help patients with account access, signups, and platform use.

Secondary Responsibilities (as qualified):

  • Lead support groups or classes for new and/or expecting parents, assigned as appropriate based on professional knowledge, credential, and skillset.
  • Perform virtual assessments with patients to identify social needs or social determinants of health (SDoH), assigned as appropriate based on professional knowledge, credential, and skillset.
 
Responsible for Handoffs:
This individual is responsible for the handoff of any patients, physician’s orders, and care plans to other Mahmee team members sharing these responsibilities on alternate shifts/workdays. When the team member is working in any assigned shift they will have the responsibility to manage patient inquiries and support needs that are submitted to Mahmee. This individual is not responsible for providing direct therapeutic care or clinical oversight -- this individual is expected to review messages that come through the Mahmee platform during their shift, answer what lies within their professional responsibilities, and reroute or refer to the appropriate care providers or resources in the platform as needed.

Requirements:

  • Certification or degree in maternal health-related topic or field (i.e. childbirth education, peer counseling, perinatal mental health, prenatal or postpartum support or education, breastfeeding/infant feeding education, etc.) 
  • Prior professional experience working in maternity, pediatric, or home health-related positions.
  • At least 1 year of experience working in a clinical or healthcare-related setting (e.g. hospital, clinic, doctor’s office, community health organization, etc.)
  • At least some experience using digital communication tools (messaging, email, video chat), and a willingness to learn new workflows and tools.
  • Excellent customer service and listening skills, and an ability to empathize and relate to persons of different ethnic and cultural backgrounds than your own.
  • A highly engaging and compassionate “bedside manner” with new and expecting parents.
  • Comfortable and capable of working alongside physicians, midwives, nurses, therapists, lactation consultants, nutritionists, social workers, and other health professionals, in a collaborative fashion that supports patients’ needs and provides a comprehensive plan of care.
  • Able to remain calm and exercise judgment in unusual or stressful situations.
  • Demonstrates the ability to organize their own work and juggle multiple assignments.

Melissa and Linda, who serves as cofounder and Chief Nursing Officer of Mahmee, are joined by cofounder/CTO Sunny Walia, an enterprise platform specialist who previously served as VP of Engineering & Operations at Demand Media and CTO at Spin Media.
